The American Urological Association has a BPH Symptom Index Questionnaire that gives you an idea if there is a prostate problem. The questionaire calculates the AUA BPH Symptom Index Score that is often used by urologists to determine the severity of prostate problems.

A score of 0-7 is mild, 8-19 is moderate, and over 20 is considered severe.
